What to Expect at an Initial Visit
If it’s your first visit to Fayetteville Psychiatric Associates, please bring the following things:
- Patient or parent/guardian valid ID (valid IDs include state issued driver license or Military ID)
- Copy of insurance card
All co-payments are due at the time of service.
For first-time patients, please arrive 30 minutes prior to initial appointment for completion of new patient paperwork. If applicable, legal guardian must present guardianship paperwork. All pediatric patients who are in the custody of Department of Social Services must be accompanied by their social worker to initial appointment.
